Course Selection Sheets

Transcript Request Form
Seniors fill out the form to have their transcripts released to applied colleges.

Student Brag Sheet
Allows students to brag about themself and their accomplishments. Guidance counselors consult the sheet when writing college recommendation letters.

Parent Brag Sheet
Allows parents/guardians to brag about their son/daughter’s accomplishments and characteristics. Guidance counselors consult the sheet when writing college recommendation letters.

Schedule Change Request Form
If student chooses to add or drop a course, student must fill out request form and have a parent/guardian signature.

Supplemental Course Drop Form
Must be filled out and signed by parent/guardian in order for student to drop a supplemental course.

Alumni Transcript Release Form
Document needs to be completed and returned to the Gudiance Department.
